Intranasal Esketamine administration in catatonia: a case report.

Introduction Catatonia is a complex psychomotor syndrome that often goes unrecognized and, consequently, untreated. Prompt and correct identification of catatonia allows for highly effective treatment prevention possible complications. Benzodiazepines electroconvulsive therapy (ECT) are the most widely studied methods. However, no uniform method has yet been brought forward previous attempts to treat on patient suffering concomitant major depressive disorder (MDD) with NMDA receptor antagonists have documented so far. Objectives To describe unknown novel management MDD intranasal esketamine, antagonist. Methods A 55-year-old woman diagnosis long-standing recurrent who was admitted psychiatric inpatient unit UniversityHospital Marqués de Valdecilla (Santander, Spain) catatonic, mutative state framed severe MDD. Different ineffective therapeutic interventions were deployed during course her illness. After failing improve under conventional pharmacological ECT, given complexity peripheral venous access this (which disabled option iv ketamine use), we decided initiate compassionate esketamine. Results Intranasal esketamine in resolution patient’s catatonic state. Clinical response from observed after 6 administrations (2-week follow-up), reaching full remission 12 sessions absence significant adverse events Conclusions Esketamine showed promising effectiveness context MDD, although further research topic needed.
